A new analysis of the large SELECT trial shows that semaglutide (Wegovy) reduces major cardiovascular events — heart attack, stroke, and cardiovascular death — equally well across all levels of frailty in adults with heart disease and obesity. Importantly, the most frail participants actually gained more quality-of-life benefit than healthier patients, with improvements in mobility and self-care. They were also less likely to stop the medication due to side effects. On top of that, semaglutide made it more likely that patients would improve their frailty category over two years and less likely that they would worsen. These findings challenge the common clinical habit of under-treating frail older adults with effective medications out of safety concerns.
