GLP-1 Drugs Protect the Heart Beyond Weight Loss — Here's How

GLP-1 receptor agonists — drugs like semaglutide and liraglutide — are known to reduce major cardiovascular events in people with obesity, but researchers have long debated how much of that benefit simply comes from losing weight. This review synthesizes evidence from landmark cardiovascular outcome trials, including LEADER, SUSTAIN-6, and SELECT, and finds that 35–55% of the cardiovascular protection these drugs provide appears to operate through weight-loss-independent mechanisms. Those mechanisms include improved endothelial function, reduced atherosclerosis, anti-inflammatory and immune-modulating effects, direct myocardial protection (strongly supported in animal models), and better microcirculatory blood flow. The findings matter because they suggest GLP-1 drugs may benefit patients regardless of how much weight they lose, and they open new avenues for targeted cardiovascular therapies.
