Metabolic HealthCKM Syndrome: A Unified Framework for Treating Heart, Kidney, and Metabolic Disease
Cardiovascular-kidney-metabolic (CKM) syndrome describes the tightly linked progression from metabolic dysfunction — obesity, insulin resistance — through kidney damage to cardiovascular disease. This review from RWTH Aachen University Hospital outlines a five-stage model endorsed by the American Heart Association, explains how albuminuria and visceral fat independently drive organ injury, and surveys the growing drug toolkit: SGLT2 inhibitors, GLP-1 receptor agonists, and non-steroidal mineralocorticoid receptor antagonists. The authors also flag the emerging role of metabolic-dysfunction-associated steatotic liver disease (MASLD) as a fourth organ system, proposing a cardiovascular-kidney-liver-metabolic framework. Barriers to real-world implementation — fragmented care, low awareness, and socioeconomic inequalities — are highlighted alongside opportunities from AI-driven risk prediction and multimodal imaging.
