NMN shields the eye's lens from fibrotic cataract by calming ferroptosis in preclinical models
A NAD+ precursor curbed lens cell scarring and iron-driven cell death in human cells and mice by activating the Nrf2/FPN1 pathway.
Riepilogo
Fibrotic cataract, including after-cataract scarring, involves oxidative stress and ferroptosis, an iron-dependent form of cell death. This study asked whether nicotinamide mononucleotide (NMN), a NAD+ precursor popular in longevity circles, could protect lens cells. Samples from patients with fibrotic cataract showed signs of ferroptosis: lower GPX4, FTH1 and glutathione, and higher ACSL4, MDA and iron. In human lens epithelial cells exposed to TGF-β2, NMN reduced lipid peroxidation, iron and inflammatory signals, restored glutathione, and reversed markers of epithelial-to-mesenchymal transition (EMT) and collagen deposition. Blocking Nrf2 with the inhibitor ML385 erased these benefits. In a mouse model, NMN restored lens structure and reduced fibrosis. The findings suggest NMN works through Nrf2/FPN1 signaling, but the evidence is preclinical and not yet proven in people.
Riepilogo Dettagliato
Cataract is the world's leading cause of blindness, and fibrotic forms, where lens cells transform and lay down scar-like tissue, make up a meaningful share of cases. Surgery is effective, but fibrosis can recur or cloud vision afterward, and no drug prevents it. Oxidative stress and ferroptosis, an iron-dependent cell death driven by lipid peroxidation, have been implicated in the process.
Researchers tested whether NMN, a precursor of the cellular coenzyme NAD+, could interrupt this cycle. They combined three approaches: analysis of clinical samples from patients with fibrotic cataract (ASC) and controls, experiments in human lens epithelial cells (SRA01/04) treated with TGF-β2 to induce fibrosis, and a mouse model of ASC treated with NMN. The cell work included an Nrf2 inhibitor, ML385, to test mechanism.
Patient samples showed lower GPX4, FTH1 and glutathione, with higher ACSL4, malondialdehyde and Fe2+, a pattern consistent with ferroptosis. In cells, NMN blocked TGF-β2-induced increases in lipid ROS, MDA and iron, and prevented glutathione loss. It reversed the fall in E-cadherin and suppressed collagen I, collagen IV and α-SMA, indicating reduced EMT and fibrosis. It also affected TNF-α and IL-6. ML240-style rescue was lost when Nrf2 was inhibited, pointing to Nrf2 dependence. In mice, NMN restored lens architecture, reduced fibrosis, and normalized iron, MDA and glutathione through Nrf2/FPN1 activation.
The work links NAD+ biology to ferroptosis control in the lens and nominates the Nrf2/FPN1 axis as a therapeutic target, possibly for preventing posterior capsule opacification or fibrotic cataract.
Caveats are substantial. This is based on the abstract only, so sample sizes, dosing, route of delivery and statistics are unknown. Cell lines and a mouse model may not reflect human disease, and no human NMN treatment data are presented.
Risultati Principali
- Patient fibrotic cataract samples showed reduced GPX4, FTH1 and glutathione, and elevated ACSL4, MDA and Fe2+, consistent with ferroptosis.
- NMN reduced lipid ROS, MDA and iron and restored glutathione in TGF-β2-treated human lens epithelial cells.
- NMN reversed EMT features, raising E-cadherin while lowering collagen I, collagen IV and α-SMA.
- Nrf2 inhibitor ML385 abolished NMN's protective effects, indicating the benefit depends on Nrf2.
- In mice, NMN restored lens structure, reduced fibrosis and normalized ferroptosis markers via Nrf2/FPN1 activation.
Metodologia
The study combined clinical sample analysis (qPCR and biochemical assays) from fibrotic cataract patients and controls, in vitro experiments in TGF-β2-treated SRA01/04 human lens epithelial cells with or without NMN and the Nrf2 inhibitor ML385, and an in vivo mouse ASC model treated with NMN. Readouts included ferroptosis markers, inflammatory cytokines (ELISA), EMT proteins (Western blot) and histology (H&E, Masson).
Limitazioni dello Studio
Only the abstract was available, so sample sizes, NMN dose and route, and statistical details are unclear. Findings come from a cell line and a mouse model, and patient samples were used only for association, not treatment. Long-term safety, efficacy in age-related cataract, and translation to humans remain untested.
