FOXO4 Senses Oxidative Stress Through Cysteines and Keeps Senescent Cells Alive via p53

Riepilogo

FOXO4 is a transcription factor that reacts to oxidative stress and helps senescent cells survive. This structured narrative review graded 89 publications by how directly they tested FOXO4 rather than other FOXO proteins. The strongest FOXO4-specific evidence covers three areas: sensing of reactive oxygen species through cysteine disulfides, stress-driven nuclear import and coactivator binding (transportin-1 and p300/CBP), and FOXO4 binding to p53 to keep senescent cells from dying. Evidence that FOXO4 itself drives antioxidant genes such as SOD2 and catalase is weak and mostly inferred from FOXO3. The senolytic peptide FOXO4-DRI clears senescent cells in mice, including endothelial cells, but has never been tested in humans. Safety, delivery and tumor-surveillance questions remain open.

Riepilogo Dettagliato

Reactive oxygen species act as signals at low levels and cause damage at high levels, and cells rely on transcription factors to match their antioxidant defenses to the redox state. The FOXO family is a central hub for this. FOXO3 has the strongest link to human longevity, while FOXO4 has been studied far less. FOXO4 matters for two reasons: it directly senses oxidation, and it helps senescent cells resist death, which makes it a possible target for senolytic therapy.

This was a structured narrative review, not a formal systematic review or meta-analysis. The authors searched PubMed/MEDLINE, Scopus and Web of Science through May 2026. Of 420 records, 300 remained after deduplication, 110 full texts were assessed, and 89 publications were retained. Evidence was classified as directly FOXO4-specific (18 papers), FOXO-family or conserved (24), or extrapolated from FOXO1, FOXO3 or DAF-16 (20). A further 27 contextual papers were tracked separately. Only direct FOXO4 evidence was treated as enough for isoform-specific conclusions.

The strongest findings concern mechanism. Oxidation of Cys477 forms a disulfide-dependent complex with p300/CBP, which acetylates FOXO4 and is later reduced by the thioredoxin system. Oxidation of Cys239 or Cys355 forms a disulfide with the import receptor transportin-1, moving FOXO4 into the nucleus; the C. elegans homolog DAF-16 uses the same mechanism. These cysteine switches work alongside Akt/14-3-3 inhibition, JNK phosphorylation at Thr447 and Thr451, SIRT1 deacetylation, and ubiquitination. In senescence, FOXO4 binds p53 and holds it in the nucleus, which blocks p53's pro-apoptotic mitochondrial activity. The D-retro-inverso peptide FOXO4-DRI disrupts this interaction. It selectively kills senescent cells in mice, counteracted chemotoxicity-induced and natural aging features, and in a vascular study lowered ROS and SASP markers and improved aortic function.

In contrast, direct FOXO4 regulation of SOD2, catalase, sestrins, GADD45 and OSER1 is not well demonstrated. These targets come mainly from FOXO3 or pooled FOXO studies, so FOXO4's role is plausible but unproven. Disease roles in cancer, muscle atrophy, cardiometabolic disease and reproductive aging are also graded, and many rest on FOXO-family inference.

FOXO4-DRI should be viewed as an emerging preclinical strategy. Human data from senolytics such as dasatinib plus quercetin cannot be transferred to it. Open questions include tissue selectivity, peptide delivery, pharmacokinetics, long-term toxicity, and whether disrupting p53 handling compromises tumor surveillance. The review calls for isoform-resolved chromatin and transcriptomic studies to define FOXO4's actual target genes.

Risultati Principali

  • FOXO4 senses ROS directly: Cys477 oxidation drives p300/CBP complex formation, while Cys239/Cys355 oxidation drives transportin-1-mediated nuclear import.
  • FOXO4 binds p53 in senescent cells, retaining it in the nucleus and blocking p53's pro-apoptotic activity, which supports senescent-cell survival.
  • FOXO4-DRI peptide selectively kills senescent cells in mice and, in endothelium, lowered ROS and SASP markers and improved aortic function.
  • Direct FOXO4 control of SOD2, catalase, sestrins and GADD45 is not proven; these targets are inferred mostly from FOXO3 studies.
  • FOXO4-DRI has no human trial data; safety, delivery and effects on p53 tumor surveillance remain unresolved.

Metodologia

Structured narrative review (not a systematic review or meta-analysis) searching PubMed/MEDLINE, Scopus and Web of Science through May 2026. Two authors independently screened and extracted data from 89 retained publications. Evidence was classified as direct FOXO4-specific, FOXO-family, or extrapolated from other isoforms, with contextual papers tracked separately.

Limitazioni dello Studio

Much of the antioxidant target-gene evidence comes from FOXO3 or FOXO-family studies, and only 18 papers were directly FOXO4-specific. The review is narrative, and its classification is partly subjective. FOXO4-DRI data are preclinical and mostly from mouse and cell models, with unknown human pharmacokinetics, toxicity and cancer-surveillance effects. The supplied text was also truncated before the pharmacology section, so conclusions on drug modulation rely on the abstract.
