Taurine and new drugs shift MELAS care from symptom control toward disease modification
A 2026 review says high-dose taurine prevented stroke-like episodes in 60% of MELAS patients, while sonlicromanol and gene editing advance.
Zusammenfassung
MELAS is a rare, devastating inherited mitochondrial disease, most often caused by a single DNA change called m.3243A>G that cripples the cell's ability to build energy-producing proteins. This narrative review surveys treatment progress published mainly between 2024 and 2026. The headline: high-dose taurine (9-12 g/day) has gained regulatory approval after a phase III trial in which 60% of participants had no stroke-like episodes. Other candidates are advancing too, including sonlicromanol, which showed company-reported signals for cognition, mood, and fatigue and is moving to phase III; zagociguat and TTI-0102, aimed at vascular dysfunction and oxidative stress; and KL1333, an NAD+ modulator in phase II. Mitochondrial gene-editing tools such as TALENs, DdCBE, and mitoARCUS can shift mutant DNA levels in cells and animals, but remain preclinical for this variant.
Detaillierte Zusammenfassung
MELAS (mitochondrial encephalomyopathy, lactic acidosis, and stroke-like episodes) is among the most clinically significant inherited mitochondrial disorders. It is most often caused by the m.3243A>G variant, which disrupts mitochondrial tRNALeu(UUR) and impairs mitochondrial protein synthesis, producing progressive multisystem dysfunction. For decades, care has been largely supportive, so any disease-modifying option matters to patients, families, and clinicians.
This narrative review synthesizes recent clinical trials, therapeutic advances, and emerging strategies for m.3243A>G-associated MELAS, emphasizing evidence from 2024 to 2026. The author searched PubMed/MEDLINE, Embase, the Cochrane Library, ClinicalTrials.gov, and the EU Clinical Trials Register.
The most notable result is regulatory approval of high-dose taurine (9-12 g/day). In a phase III trial, 60% of participants had complete prevention of stroke-like episodes. Sonlicromanol (KH176) showed signals of improvement in cognition, mood, and fatigue in phase IIb data, which were company-reported, and is advancing to the phase III KHENERFIN trial (NCT06451757). Zagociguat and TTI-0102, which target vascular dysfunction and oxidative stress, are in ongoing trials. KL1333, an NAD+ modulator, is in phase II (FALCON). Preclinical genome-editing tools (mitochondria-targeted TALENs, DdCBE base editors, mitoARCUS nucleases) shifted heteroplasmy in patient-derived cells and animal models.
The review frames the field as moving from symptomatic care toward mechanism-based disease modification, with taurine as the first regulatory-approved disease-modifying therapy and late-stage small molecules and genome editing following.
Important caveats apply. This is a narrative review assessed from its abstract alone, so trial designs, sample sizes, and safety data are not available here. Sonlicromanol results are company-reported, and the author states that clinical applicability of gene editing to m.3243A>G MELAS needs further investigation.
Wichtigste Erkenntnisse
- High-dose taurine (9-12 g/day) gained regulatory approval as the first disease-modifying therapy for m.3243A>G MELAS.
- In a phase III trial, taurine completely prevented stroke-like episodes in 60% of participants.
- Company-reported phase IIb data suggest sonlicromanol improves cognition, mood, and fatigue, supporting the phase III KHENERFIN trial.
- Zagociguat, TTI-0102, and the NAD+ modulator KL1333 are in ongoing trials targeting vascular dysfunction, oxidative stress, and energy metabolism.
- TALENs, DdCBE base editors, and mitoARCUS shifted heteroplasmy in cells and animal models, but clinical use for MELAS is unproven.
Methodik
Narrative review focusing on evidence published from 2024 to 2026 on m.3243A>G-associated MELAS. Sources searched were PubMed/MEDLINE, Embase, the Cochrane Library, ClinicalTrials.gov, and the EU Clinical Trials Register. The abstract does not describe a systematic selection or quality-appraisal process.
Studienlimitierungen
Only the abstract was available, so trial sizes, designs, controls, durability of benefit, and safety are not assessed here. The sonlicromanol data are company-reported, and a narrative review lacks systematic methods and may be subject to selection bias. Genome-editing evidence is preclinical, and its applicability to patients with m.3243A>G is uncertain.
